Welcome to My Horizon!

At My Horizon we listen, and focus on having fun and friendship while working on goals. We provide a safe, accepting, and positive learning environment for living skills, social skills, exercise, nutrition, computer skills, arts & crafts, and many other activities, and always keep in mind, its supposed to be fun!

Supported Employment (Employment Services)

Employment services assist recipients to become gainfully employed (including self-employment) in a job that meets their career goals. Employment services include pre-employment and supported employment services that are provided over a specified period of time, consistent with the person-centered approach to these services. Individuals are encouraged, on an ongoing basis, to explore their interests, strengths, and abilities relating to employment or career advancement.

Day Habilitation

Day habilitation services may be provided to assist recipients to acquire, retain, and improve the self-help, socialization, and adaptive skills necessary to live successfully in home and community-based settings. These services must provide supervision and a secure environment for recipients, may be planned to reinforce skills or lessons taught in other settings, and may include both individual and group activities.

Habilitation Services

Residential habilitation services may be provided to assist recipients to acquire, retain, and improve the self-help, socialization, and adaptive skills necessary to maximize independence and to live in the most integrated setting appropriate to the recipient’s wishes and needs. At My Horizon, residential habilitation services are rendered as family home habilitation services, supported-living habilitation services, or in-home support habilitation services.

Respite Care Services

Respite care services may be provided for primary unpaid caregivers and providers of family home habilitation services that are in need of relief or will be unable to provide care for recipients for limited periods of time, if those caregivers provide the oversight, care, and support needed to prevent the risk of institutionalization of a recipient by assisting with basic personal activities or with activities related to independent living.
