My Horizon’s individual skills development program consists of five phases. These phases are designed for ongoing improvement of individual skills.

The initial and most important goals and objectives will be prescribed by the Plan of Care. Related skills are identified by the Skills Assessment Baseline Survey. The proper context for the skills sought will be provided for the individual through a variety of enjoyable activities. This approach follows the concept that an individual learns best while having fun, while maintaining a focus on skills development.
At the end of each week, a skills assessment will be made for each skill being developed. The weekly results will then be compiled into a monthly progress report and ultimately an annual progress report. Parents, providers, and participants will all be contributors to this interactive program. |
